1. Continual assessment of patients and their carers needs, when those needs are complex and change across the caseload. 2. Undertake nursing interventions (such as providing personal care to patients, administration of prescribed medication through a variety of routes, tissue viability care, ensuring infusion pumps are working correctly) consistent with evidence-based practice, transferring and applying knowledge and skills to meet individual patients needs. 3. Provide psychosocial support to patients in emotional distress and their distressed families and provide a high standard of evidence based information to them. 4. Communicating effectively with multi disciplinary health care professionals, medical and social work colleagues, patients, carers and managers across organisational boundaries. 5.
