Full Job Description

We are looking for an enthusiastic Registered Nurse Associate (RNA) to join our innovative Virtual Ward Team as a Discharge Clinician. This is an exciting opportunity to contribute to a cutting-edge healthcare model, enabling patients to receive hospital-level care in the comfort of their own homes. As part of the Virtual Ward Team, you will play a vital role in ensuring smooth and safe discharge from acute hospitals for patients, supporting their transition to ongoing home-based treatment.
Be a part of our forward-thinking Virtual Ward Team and help us redefine patient care!, This role is an opportunity for a Registered Nurse Associate with NMC registration and an appropriate Foundation Degree.
+ Assess and prepare patients for discharge from the acute hospital onto the Virtual Ward (VW), ensuring they are ready for a safe transition to community care at the place they call home.
+ Collaborate with multidisciplinary teams, including doctors, nurses, and allied health professionals, to identify patients suitable for the Virtual Ward, identifying clinical needs and liaising with the VW Hub coordinators.
+ Be able to consider a range of factors regarding service capacity/occupancy and consider this when reviewing potential patients to meet clinical priorities/consider acute flow pressures/VW availability.
+ Work together with the VW Hub to complete onboarding processes and handover care and escalation plans.
+ Coordinate necessary equipment, medications, and any care/AHP support to facilitate timely and efficient discharges onto the Virtual Ward caseload.
+ Provide education and support to patients and their families on the VW service.
+ Ensure accurate documentation of patient records, discharge plans, and communications with healthcare professionals., + The healthcare provider of first choice for the people of East Sussex.
