Working as a Clinical Support Worker, the post holder will be expected to apply a rehabilitation approach to all aspects of their role; promoting functionally independence in all activities of daily living.
The post holder will undertake roles and duties delegated by a suitably qualified and regulated professional, including nursing and therapy staff.
They will work collaboratively with the team to meet the needs and co-morbidities of patients; following structured treatment plans for rehabilitation.
The post holder will continuously and autonomously review and assess patient's level of functional ability.
They will liaise with appropriate professionals to support the ongoing rehabilitation needs of the patient, progressing and adapting treatment plans under guidance.
The post holder will follow practice policy and procedures and will champion standards of rehabilitation and care across the service., To act in a professional manner and ensure Trust standards are maintained and monitored to improve the quality of care to all who come into contact with the services provided by South Warwickshire University NHS Foundation Trust.

Ensure records and documentation are maintained, kept confidential and meets Trust standards in line with the Data Protection Act.

To report any accidents or incidents as described by the Trust's policy and procedures ensuring an open and transparent culture.
To take reasonable care of the health and safety of themselves and of other persons.

To assist other wards, departments and teams within the Trust as and when required by service needs.
To adhere to the Trust's IT policies regarding e-mail and internet usage.
To ensure children and adults are protected from abuse and avoidable harm and report any safeguarding concerns in accordance with Trust policies and procedures.
Clinical Practice
To accurately and promptly carry out the instructions of the registered professional who is professionally accountable for the delivery of care.
To assist in the supervision of other healthcare and support staff as directed.
To report to the registered professional any patient who gives cause for concern or any observed changes in the client or patient's condition.
Provide and deliver a high standard of individualised care under the direction but not direct supervision of the registered professional.
To report all care that has been given to patients within the team to a registered professional.
To accurately document all care delivered to patients, maintaining confidentiality at all times.
To assist in the preparation of patients for treatment, investigations or procedures as directed by the registered professional.
To support relatives demonstrating compassion and empathy under the guidance of a registered professional.
To assist with the discharge process for patients as directed by the registered professional.
To gain informed consent for treatment in accordance with Trust policy. This will frequently be to patients/carers and relatives who may have diverse or complex emotional, physical and psychological conditions e.g. deafness, dysphasia, depression, chronic pain, etc. and will require use of verbal and non-verbal communication tools.
To champion standards of rehabilitation, working towards patient-centred goals to promote improved functional ability and independence.
To deliver Physiotherapy treatment programmes using guidelines set by qualified Physiotherapists. E.g. patient education, exercise programmes or classes.
To deliver treatment programmes using guidelines set by qualified Occupational Therapists to support patients E.g. Washing & dressing, transfer assessments, use of ADL equipment.
Evaluate patient progress, reassess and if necessary following discussion with qualified Therapists alter treatment programmes within scope of practice.
To ensure own competency prior to using any medical device e.g. equipment, hoists and mobility aids.
Maintain accurate documentation both written and electronic in accordance with legal and Trust guidelines.
To hold responsibility to all patients and work without direct supervision.
To maintain a clean, tidy and safe environment and equipment Trusts premises, having due respect for Health and Safety.
To use and apply clinical knowledge and experience to assess and manage clinical risk within the limits of own scope of practice.
To work within infection control guidelines to ensure that therapy/treatment/care does not constitute a risk of infection either to the healthcare professional to the client or to any persons visiting/working in Trust premises.
To support clinically other members of the team accordingly.
To collect clinical data relevant to the service, as required.
To achieve and maintain necessary competencies require to fulfil the role.
Communication
Welcome patients, relatives and visitors to the clinical area, providing assistance, support and information where required. Being aware of barriers to effective communication and the means to overcome them.
To communicate effectively with patients, carers and relatives on an individual or group basis to maximise rehabilitation potential through verbal and non-verbal methods. This may require explanation, instruction, persuasion, motivation, negotiation and reassurance.
Always introduce yourself in line with the Trusts privacy and dignity promises and the principles of good customer service.
To answer the telephone when necessary in a polite, courteous manner and pass on enquiries to the registered professional.
Communicate effectively within the multi-disciplinary team and other departments to ensure that patient care is well planned and coordinated.
To be committed to a team approach in order to provide continuity and efficiency of service provision.
To liaise with multidisciplinary team/colleagues/medical nursing staff as required.
Attend formal handovers and receive frequent updates on patient programme plans.
Report incidents, accidents and complaints as per Trust Policy and to the senior professional on duty, ensuring an open and transparent culture.
Use the Trust information support system and other computer systems.
Education, Training and Personal Development
To ensure attendance at mandatory training sessions, i.e. infection control, resuscitation and manual handling training.
To complete the Trust's Clinical Support Worker/Healthcare Assistant Induction programme.
To participate in appraisals and personal reviews and work to achieve agreed set objectives.
To undertake training as necessary for the development of the post and the relevance of the department including IT and clinical skills.
To continue personal development in relation to identified objectives.
Maintain up to date knowledge of changes in policies and procedures relevant to the post.
Other
To identify opportunities to improve standards in order to provide the best patient care within the available resources.
Take all reasonable steps to ensure the safe keeping of patient's property in accordance with Trust policy.
Promote a professional, welcoming and uncluttered environment; keep all storage areas safe, clean and tidy.
To share responsibility in the care and maintenance of all equipment in the clinical area including all equipment cleaning.
Ensure all waste and soiled linen is disposed of in line with Trust policy.

Campion is a 22-25 bedded unit supported by GP. We care for patients who require sub-acute admission prevention, dementia care & rehabilitation or palliative care.
This new and exciting position is suitable for anyone looking to 'develop' their career as a Rehabilitation support worker. You will be part of the multi-disciplinary team and have the ability to work flexibly/autonomously and efficiently with good organisational skills under the guidance of qualified therapists that enable you to work on your own initiative contributing to the smooth running of a very busy department.
Your role will be to support and assist healthcare professionals such as nurses, doctors and other integrated professionals in the delivery of quality patient care and the effective rehabilitation of our patients. You also must have excellent communication and interpersonal skills, be reliable and work well in a team.
At interview you will be expected to demonstrate knowledge of the importance of rehabilitation of our patients although you can expect further training and if successful will have competencies to meet going forward.

Probationary Period
