Full Job Description

We are looking for someone with a passion for community rehabilitation who will be key in supporting the occupational and physiotherapists to deliver effective rehabilitation programmes during a person's episode of care. You will develop and maintain relationships that integrate health and social care to support and reduce the need for hospital admission for patients, ensuring a they maintain their ability to engage in daily activities. This is a great opportunity to consolidate your clinical skills supported through your personal development and appraisal plan., Working as a member of the Community Independence Service you will contribute to a person's episode of rehabilitation to maintain or improve their independence to continue living in their primary residence with as minimal support as possible
You will work under the guidance of health care professionals to assist therapists by carrying out delegated aspects of patient care without direct supervision and reporting back patient's progress or issues to the relevant therapist.
You will undertake specific skilled support work to aid the rehabilitation of patients in the community or the management of patients at home to prevent imminent unnecessary hospital admission.
You will be responsible for ordering equipment and department supplies, ensuring that these are received in full and good working order, chasing any outstanding items or actions as required
