Full Job Description

  • Undertake Immediate Needs Assessments

  • Implement a rehabilitation program with a full MDT based on the client's individual needs and goals

  • Provide Case Management support

  • Advocate for the client's needs while building strong working relationships with families and instructing solicitors

  • Produce action plans and SMART goals with timeframes that are specific and relevant to the client

  • Compile and lead the MDT

  • Coordinate with NHS and Private providers, such as physicians, nurses and other healthcare professionals

  • Recruit, select and manage a team of care/support workers for clients

  • Compile risk assessments on key activities as required and identify risk management strategies that will manage the safety of all parties in a positive manner

  • Evaluate the amount and quality of care provided by the care provider and implement improvements as needed

  • Ensure compliance with all relevant regulations and standards

  • Complete file notes of all contacts daily into the specific Case Management software, A specialized team of health care professionals whose focus is to provide specialist knowledge, skills and resources for those with a spinal injury, orthopaedic injury, limb amputation or acquired brain injury.


  • Case Management provides a complete service for injured individuals navigating through and providing structure to a legal case and beyond settlement.

    SICM Ltd also provide assistance to clients employing their own personal carers and domestic assistants. This includes HR, management of staff and payroll services.

    We have an exciting opportunity for a Case Manager with a minimum of 12 months experience to join our friendly Spinal Injury Case Management Team. We are a Yorkshire based company supporting clients across several regions across the UK. As a busy, caring team of Case Management Specialists, we are passionate about providing a non-discriminatory service to people with spinal cord injury and other disabilities. We work with children and adults of all ages in a domiciliary setting. We are now seeking hard-working individuals to cover East Midlands.

    For this role you will hold a professional health qualification gained in either nursing, social work, occupational therapy or physiotherapy which requires you to be HCPC, NMC or Social Worker Registered. You will also be a driver with access to your own vehicle and the ability to travel.

    Our Case Managers follow a full induction and training program via formal teaching and peer mentorship.,
  • A minimum of 12 months experience working as a Case Manager is essential

  • UK professional health qualification which requires you to be HCPC, NMC or Social Worker Registered

  • Experience supporting people with neurological conditions, in particular Spinal Cord Injury, in a community setting - this could be adults or children

  • Excellent communication and interpersonal skills to effectively interact with clients, families , lawyers and healthcare professionals

  • Good understanding of the litigation process

  • A high degree of self-motivation

  • Strong organisational skills with the ability to prioritise tasks and manage time effectively

  • Strong leaderships skills with the ability to lead, motivate and inspire a team

  • Flexible and positive approach to working with other community professionals

  • Desirable knowledge of the CQC fundamental standards

  • Desirable Knowledge of the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS) and Safeguarding frameworks

  • Proficiency in using office software such as Microsoft Word, Excel and Powerpoint

  • Excellent report writing skills

  • Full Driving Licence and access to a car

    Pay rates will be discussed individually with applicants and is dependent on skills and experience.
