Full Job Description
The Rehabilitation Coordinator will work as an integral member of a highly skilled multidisciplinary team to co-ordinate the rehabilitation patient pathway from admission to discharge, during this secondment. Their remit will cover all patients admitted to The Royal Sussex County Hospital, Brighton with complex rehabilitation needs following major traumatic injuries. This patient group will include all major traumatic injuries, as dictated by National Major Trauma Registry (NMTR) and the Sussex Trauma Network criteria.
They will work across the organisation to co-ordinate the care pathway of all patients who require specialist rehabilitation. This would include coordinating the patient's care while in the acute setting and assisting in facilitating the transfer to appropriate interim facilities. They will actively participate in relevant Trauma network meetings.
The post holder will also work closely with the Major trauma practitioners, major trauma MDT and major trauma operational team to ensure that comprehensive data systems are in place to provide the trust with informatics on rehabilitation activity., + Serve as the primary contact for rehabilitation matters for all major trauma patients during their hospital stay.
+ Collaborate with existing teams to ensure smooth transfer to appropriate rehabilitation services within the Sussex network.
+ Act as a liaison for patients, and other health professionals regarding rehabilitation issues.
+ Organise and manage the complex care and rehabilitation pathways for major trauma patients.
+ Innovate and implement service development projects.
+ Drive excellent patient care and advocate for patients.
Responsibilities:
+ Identify and support the ongoing specialist rehabilitation needs of major trauma patients from early admission through post-discharge follow-up.
+ Ensure all major trauma patients have a completed rehabilitation prescription in line with national standards and verifying completion before discharge.
+ Implement and audit key performance indicators related to trauma patient management in rehabilitation.
+ Provide information and advice to the Major Trauma Service, inpatient teams, patients, and relatives., + Attend appropriate meetings regarding patient discharges and Trauma/ Rehabilitation speciality group meetings.
+ Maintain excellent communication to ensure the accurate and timely dissemination of information. This includes adherence with the rehabilitation prescription process, developing reports for MDT/ Speciality Care Groups.
+ Develop and maintain data base/ information system to demonstrate timing and co-ordination of rehabilitation decisions, referrals and transfer of care.
+ Identify current bottlenecks and gaps in service and implement internal process solutions and contribute to external developments.
+ Take responsibility to ensure that appropriate training and development needs are identified and implemented to support these activities.
+ Ensure referral pathways are agreed by all relevant internal and external stakeholders.
+ Document and feedback via Trauma/Rehabilitation Speciality groups, all relevant information influencing referral pathways, network forums and funding restrictions.
+ Maintain accurate and comprehensive daily patient records to include any variances from planned activity and maintain MDT communication regarding the status of rehabilitation referrals.
+ Support other staff when indicated in the management of challenging patients.
