Full Job Description

We have an exciting opportunity for Therapy Rehabilitation Practitioner to join a new intermediate care at home therapy service within the Wigan borough. This is a full time post (37.5 hours) working across Monday to Sunday., The post holder is required to work without direct supervision, undertaking assessments and decision making within agreed protocols, underpinned by appropriate experience / knowledge.

To promote and sustain independence/wellbeing for the individual in the relevant community setting through a holistic approach that encompasses patient values and beliefs.

To work autonomously and carry out specific delegated clinical rehabilitation tasks and responsibilities, taking responsibility for their own workload.

The post holder will use technical and creative skills to provide therapeutic activities for individual patients and will be responsible for review and progression of treatment programmes and interventions within guidelines and specific timeframes.

The work can take place in a variety of settings including, community units, patients' own homes and other community settings, alongside other team members but as a lone worker in community at times, following the Organisations Lone Worker policy.

Function as an effective member of the multi-disciplinary team and the integrated team within Health and Social Care.

We are aiming to recruit a suitably qualified, enthusiastic and engaged individual to our new Intermediate Care at Home Team.

The Team will provide rehabilitation for patients in their own place of residence, and other community settings. Joining up the services provided by the 'Home First' Team, in addition to Community Therapy, whilst working closely and flexibly with social care colleagues and the existing Intermediate Care Therapy Team who deliver rehabilitation into our existing bedded units.

The ideal candidate will have an open minded and flexible approach, have a 'can do' attitude and be willing to come into a new initiative to support the development and establishment of the service - have excellent communication skills, Function as an effective member of the multi-disciplinary team and the integrated team within Health and Social Care and also the ability to comfortably work across role boundaries.

It is essential for this role that you have the ability to independently travel across the borough to assess, formulate therapy plans and deliver therapy interventions for patients in their own place of residence. You must have relevant business insurance for your vehicle.

UK NHS experience is essential.
