Rehabilitation Support Worker

Come and join our friendly, sociable, valued and experienced team working within an integrated Health and Social Care community rehabilitation service based in Eastbourne, covering the Eastbourne Hailsham & Seaford area.
The JCR service is a short-term service which assesses and treats people within their own home. The service aims to prevent unnecessary admissions to hospital or long-term care, enable earlier discharges from hospital and promote independence and quality of life., You will be part of a friendly, supportive, professional integrated team which provides a 7-day service and uses an inter-professional model of working. Continuing Professional Development is supported and supervision and training are integral to the working practices of the team. You will key work non-complex clients, provide rehabilitation to people with a variety of conditions, and respond where appropriate to urgent referrals supporting clients to stay in their own home., The post holder will be required to work flexibly and visit people in their own home and liaise with clients in acute and intermediate care settings if necessary prior to discharge. The ability to travel around the county in a timely fashion is essential.
