Full Job Description

ChippenhamCommunity Team have an exciting vacancy for a Rehab Support Worker. The role ofa Rehab Support worker is to work closely with a wider team to supportindividuals in their own homes for a short period after discharge from hospital. We provide support and rehabilitation to promote independence in activities ofdaily living. The role also includes supporting our therapy team with homeexercise programmes and provision of equipment, following care plans andproviding feedback. Thesuccessful candidates will need to be able to provide clear evidence of havingpreviously worked in a care role, preferably with older people or people withlong term conditions. We would either like you to have a Care Certificate andNVQ Level 3 apprenticeship or willingness to study to that level. We have a range of trainingand development opportunities, including an induction program and ongoingeducation. Due to the strong emphasis on rehabilitation,training in rehabilitation focused care will also be provided. Applicants need to have enthusiasm andcommitment to working within a community-based team and to have recentexperience of working in or in partnership with community services within theNHS or Social Care., Access to a vehicle that can be used for business purposes and hold a full and valid driving licence., Essential
