Full Job Description
The candidate will be working within the community beds team to implement rehabilitation and ongoing care programmes to patients, you will be expected to carry out goal focused programmes designed by and under the supervision of the clinical team, with the aim of helping patient to return to their own homes after a period of hospitalisation, illness or not coping. You will work with a varied caseload of patients, where no two days are the same. The candidate will be expected to work between our three care homes across the city.
The community beds team is a dynamic multidisciplinary team, including physiotherapists, occupational therapists, nurses, mental health nurses, assistant practitioners and rehabilitation support workers. We have multiple beds within three care home, patient are discharged from the local acute trusts into the beds for a period of rehabilitation and/or assessment, to determine their long term needs., The candidate will undertake rehabilitation and evaluation of clients care programmes alongside a clinician or independently in consultation with a senior member of the team , you will need to be able to use your skills to motive patients to achieve the desired outcomes and goals.
Communication will be a key quality of the successful candidate, you will need to be able to communicate effectively with patients and occasionally family member, you will also be required to communicate well with other of members of the team.
