Full Job Description

+ To coordinate referrals, appointments and communications for patients across the 3P clinical pathways.
+ Liaise with the referrers, clinicians and patients to ensure that appointments are used effectively and highlight the need for extra clinics/lists to cover changes in demand and capacity.
+ Assist in co-ordinating patient bookings, activity and special requests within the 3P work streams.
+ Assist in the preparation for patients to be discussed at each 3P clinical work stream meetings and MDT's and tracking of patient's progress across the pathways.
+ Actively engage in the wider booking and re-booking process.
+ Update and maintain all databases in a timely fashion ensuring accuracy of data.
+ Assist with the monitoring of waiting times targets and escalate risks relating to patient pathways and waiting times with the Service Delivery Manager and the 3P work stream leads.
+ Collect and record relevant data and update the management team and 3P work stream leads on a regular basis.

We are looking to appoint a Pathway Administrator to support the 3P clinical pathways and work streams for Renal & Transplant patients in the West London Renal Transplant Centre (WLRTC). The post holder will primarily help provide a seamless service for patients referred across the Accelerated Renal Pathway, Accelerated Care Plan, and Dialysis in Diabetes pathways.
You will be able to work both autonomously using your own initiative and within the team in the development and delivery of the 3P programme in managing the patient flow. This role entails working collaboratively within the renal department's administration team as well as providing the support to the clinical teams across the 3P work streams.
You will be responsible for managing the day to day organisation and smooth function of the 3P clinical pathway utilizing designated operational practices within the bookings administrative team. Therefore, it is expected that you will be computer literate, an excellent communicator with the ability to build relationships across staff groups. You will be a highly organized individual working with attention to detail and able to manage competing priorities and use your own initiative to solve problems.
