We have ambitious plans to shape how we work with families, placing practitioners and relationship-based practice at the heart of what we do. Through Vital Signs, our vision for Children in Norfolk, we work with whole family networks and across the whole system to support families to build on their strengths and to achieve the best possible outcomes. We bring systemic change to working with the hardest to reach young people, engaging and placing adolescents, and fundamentally challenging traditional methods of practice and theoretical thinking. Bringing together a team of specialists through a central hub, our new model is designed to provide intensive short-term interventions to prevent entry into care, escalation to long term care, prevent placement breakdown or facilitate placement step downs. We are looking for a full time Residential Hub Worker Portfolio Lead to proactively provide the support and interventions families and young people need so they can achieve the best possible outcomes in life. You will contribute to the care planning process by participating in statutory reviews, planning meetings, and progress meetings, will undertake 'sleep-in' duties on a rota basis and will provide outreach support for young people and families. You will be involved in the induction and mentoring of new staff, providing expertise and guidance.
With an NVQ level 3 in Health and Social Care, you will have previous experience of direct work with families and young people. You will evidence the ability to form effective relationships with young people, be a good listener, have patience, understanding, humour and passion to support young people within the No Wrong Door service.
