Full Job Description

You will be responsible for supporting the Service and Deputy manager at our ABI Residential service in Bramley, Leeds - Summerfield Court, and be the point of contact for the rest of the team when the Locality manager is off site. You will be carrying out care needs assessments, creating and maintaining person centred support plans and ensuring high quality support is being provided to each of the people that we support along with the staff teams. Carrying out other duties designated by the Manager which may include delegation of duties to support workers. You will always promote good practice and act as an effective and positive role model. Your working hours will be a mix of on rota and off rota time. During your on rota time, you will be required to work alone with the people you support or as part of a team, and may be required to provide support in any location appropriate to meet the needs of the service.,

  • You will have overall responsibility for the development and implementation of individuals' support plans, including assessment and relevant risk assessments for new People we support.

  • You will encourage the people you support to take an active role in all aspects of daily living, and ensure the appropriate level of individual care and support is identified;

  • Manage the review and maintenance of all support plans to ensure the ongoing suitability of the support that is being provided, including daily outings, holidays or recreational pursuits.

  • Assisting with individuals' care, including personal care and assisting with bathing in order to maintain a high standard of hygiene where the individual requires this. This may include people moving techniques, using hoists and repositioning individuals as required to maintain health and safety requirements

  • Participating in and assisting to plan and arrange individuals' daily outings, holidays or recreational pursuits when required in order to assist the people you support with all aspects of their daily life.

  • Building working relationships and engage in positive communication with individuals, families, advocates and other professional colleagues to support individuals as required

  • Undertaking and/or assisting individuals with household activities such as cooking, mealtimes, laundering and cleaning in order to promote a healthy environment for the people you support and the promotion of independent living.

  • Recording and completing any appropriate documentation in order to ensure that individuals' records, observations, or other documents are accurate and informative. Compiling reports for contractual or company compliance may also be required. You will provide this information upon request to the Locality Manager.

  • Ensuring that all people we support have up to date medications and MAR charts, in accordance with the company's Administration of Medication Policy; including staff medication competencies and regular audits of Medication records

  • Maintaining compliance and support of staff through coordination and completion of supervision, appraisal and team meetings and spot checks as per Company policy including participating in the recruitment and induction of new staff.

  • Attending, participating and contributing to relevant multi-disciplinary meetings for the people that we support.

  • Ensure High standards of care and support are delivered through pro-active auditing.

  • Organise, attend, participate and contribute to team meetings when required to promote communication within the team.

  • Ensure good governance through participation and completion of the auditing process

  • Ensure staff teams are supported and managed through good leadership and management paying attention to the completion of supervisions.

  • Undertaking all training (including statutory) relevant to the needs of the service and the people you support and according to relevant legislation, in order to professionally develop and gain skills beneficial to the individuals.

  • Ensuring all policies and procedures applicable to the service and the company are followed and understood in order to comply with regulations and good practice.

  • Working shifts and visits on a staffing rota including weekdays, weekends, split shifts, wake nights, sleep-in and bank holidays as required, and at multiple sites.


  • You may be required to undertake other duties commensurate to your role and/or working hours, as may reasonably be required of you at any other sites or premises, locations in the community, and within the homes of people we support. The Company may reasonably require you to meet the changing needs of service provision.
