Full Job Description
An exciting and unique opportunity has arisen to be a part of the Mental Health Intensive Support Service, based in Chester.
This service provides specialist assessment, treatment, interventions & support, to help people recover from complex mental health problems & to regain the skills & confidence to live successfully in the community., The Resource Co-ordinator will be responsible for the planning and time management of the locality teams across Community Rehab. Including ensuring the Lone Working policy is followed and conducting safe and well checks.
You will be responsible in support of key workers to respond to changes and challenges to the time management of the team and contribute to the collaborative planning and review of recovery goals with service users and team members.
You will continue to work with service users as a Resource Co-ordinator and support colleagues in this role.
CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
