Do you have a sound knowledge of respiratory conditions and how integrated working can be usedto support this client group?Are you passionate about empowering your patients?More emphasis on community-based care means there's never been a better time to join ourteam. Having been awarded as 'Outstanding' in a recent CQC inspection, we are Kent's leadingprovider of NHS services in the community and we are looking for a highly motivated respiratoryspecialist practitioner to join our specialist respiratory team in East Kent.If you successfully secure this post you would work alongside an established multi-disciplinaryteam and in addition hold a caseload of complex patients with a range of respiratory conditionsthat require specialist input. You would autonomously manage the assessment, planning, deliveryand evaluation of patient care and utilise a whole systems approach when supporting admissionavoidance for this client group., As a specialist practitioner, you'll deliver excellent patient care along a care pathway acrossorganisational boundaries; your role will involve acting as a specialist resource and providingeducation to the wider multi-disciplinary team across the health economy. This will include puttingin place management plans for patients who have complex respiratory conditions.In return for your hard work we can offer you a flexible working environment; personaldevelopment and a comprehensive corporate and local induction programme. and we will consider a band 6 or aband 5 development looking to achieve a band 6 role if a passion for respiratory care isdemonstrated. We would consider part-time working of 30 hours per week also if requested withworking pattern dependant on service provision.We as a trust will support with learning, development and progression.
