Full Job Description

  • Work as a team member within the 2 specialities contributing to the delivery of the specialist services, supporting other team members to manage their workload and treatment of our patients.
  • Assess patients, plan and implement care, provide specialist advice; maintain thorough records: both in the hospital and community settings.
  • Provide support and education to other staff/students.
  • Manage nurse led telephone and face to face clinics.
  • Attend the weekly MDT's for the Services: taking the opportunity to present cases, and learn from other cases discussed with the consultants present.
  • To provide expert nursing knowledge, skills and experience in the field of pulmonary fibrosis and pulmonary hypertension and act as a resource to advise, educate and support those involved in the care of people with these conditions, in both primary and secondary care.
  • Administration responsibilities to include audit, quality measures and activity analysis.
  • Work collaboratively and co-operatively with clinical colleagues to develop services and always seek to improve the quality of care delivered.

    The applicant must be enthusiastic with excellent communication skills. They will be working within the established team andwill be providing face to face clinics , home visits and ward reviews so must have the ability to work autonomously.
  • The post will enable the applicant to develop new skills and knowledge within the area of pulmonary fibrosis and pulmonary hypertension, working across the acute trust and community setting. They should be able to work independently providing high quality care and support to the patients and their families. This is a developmental role depending on experience. The successful applicant will be started at band 6 AFC and be expected to complete the non-medical prescribing course (if not already completed). Once complete and fulfilling the band 7 criteria they will be elevated to that banding. Your contract of employment is subject to you undertaking and successfully completing the non-medical prescribing coursewithin 2 years of taking up the post.
