Respiratory Specialist Nurse / Physiotherapist

You will work closely with Portsmouth Hospitals University NHS Trust, primary care, and other community care services to facilitate safe and effective care for patients.

The team provides urgent care to people in their homes which helps to avoid hospital admissions, supports earlier discharges from hospital and enables people to live independently for longer.

We also provide virtual wards which enable a safe alternative to hospital for patients living with long term respiratory conditions, allowing them to receive holistic care in their own home.

Join today to help us meet the needs of patients and their families, supporting them to achieve improved health and wellbeing., Undertaking a Specialist Respiratory Nurse / Physiotherapist role in leading, planning and co-ordinating the delivery and evaluation of care across the Community Respiratory and respiratory admission avoidance services.

Providing specialist Respiratory Nursing / Physiotherapy interventions to patients as part of the specialist teams, assessing and treating own caseload of patients who may have a complex presentation and maintain associated records as an autonomous practitioner.

Meeting the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive and evidence-based and which promotes independence.

Deputising for the Respiratory Team Lead including co-ordinating the day-to-day work, influencing clinical decisions, and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice, underpinned by theory and expertise.

Prioritising and undertaking initial assessments, providing specialist advice and expertise in line with the requirements set out in the HCPC / NMC code.

Critical analysis of complex clinical data, such as arterial/capillary blood sampling to inform management and decision making.
