An exciting opportunity has arisen to join an innovative and dynamic team within the Rehab and Frailty Team (RAFT) in the community. We are looking for an experienced and motivated Band 5 Registered Nurse who is passionate about service development and providing high standard of patient centred care. You will be working closely with a friendly team of Multidisciplinary staff including Doctors, Nurses, Occupational therapists, Physiotherapists, IHCA's, HCA's and rehabilitation assistants who are committed to maintaining independence, promoting self-care and assisting patients to achieve their goals. The successful candidate will be hardworking, reliable and able to use their initiative and work flexibly to respond to the service needs.
The Rehab and Frailty Team covers the hours 07.00 - 22.00 seven days a week 365 days a year. We are committed to upskilling and supporting peoples professional development.

Main duties of the job
A passion for care and the patience for teaching are also important qualities within this profession as the work includes educating both patients and family on the basics of healthcare and self-management/shared care.
Professionals in the Rehab and Frailty Team work varied shift patterns, to cover the team operating hours from 7am - 10pm, Monday -Sunday.
Our patients will usually include elderly people with frailty and long-term chronic conditions. Also physically disabled people, including any complex care needs from age 18 upwards including those with terminal illnesses.
The daily workload for a Frailty Community Nurse is wide ranging from multiple assessments for holistic care. Also, clinical nursing procedures, emotional support to helping patients and families to manage their condition.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
High Cost Area Supplement - Fringe
This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,258 and a maximum payment of £2,122 per annum pro rata).

+ End of life care and Advanced Care Planning
+ Managing the deteriorating patient with frailty
+ Complex and simple wound care,
+ Observations and ECG's
+ Catheterisation
+ Venepuncture,
+ Utilising advanced physical assessment skills
+ Pressure area care and ordering pressure prevention equipment
+ Comprehensive Falls assessments
+ Taking Medical and Social History
+ Completing Comprehensive Geriatric Assessments (Including Clinical Frailty Scoring and Memory Assessments)
+ Attending MDT's and Broad Rounds
+ Liaising with other community teams and acute trusts
+ Line management of Junior members of Staff
+ Health promotion and education
+ Referring to Therapists for patient rehabilitation
