Full Job Description

An exciting opportunity has arisen for a motivated enthusiastic Registered nurse to work as a part of the rheumatology services. To provide information, advice and support to patients with a rheumatological condition who are being monitored, until established on DMARD therapy., The role requires running own clinics and also giving telephone advice on an ad hoc and rostered basis, recognising when onward referral/advice is needed. The nurse has a pivotal role in supporting multidisciplinary teams and coordinating the pathway for patients. This is a developmental role and there is an expectation that the post holder will continue their own professional development in line with personal and departmental requirements., The post holder will be responsible for assessing, monitoring and reviewing patients with a rheumaological disease and who are receiving Disease Modifying Anti Rheumatoid drug (DMARD) therapy including oral biologic treatment.

To ensure that all patients receiving DMARD treatment are safely monitored in a timely manner, and according to local and national guidelines, and transferred to shared care as soon as is safely possible.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
