Full Job Description

An exciting development opportunity has arisen for an experienced nurse or allied health professional to join The Richmond Response and Rehabilitation team as Service Manager.

The Richmond Response and Rehabilitation service, established 2014, is an integrated health and social care service, focused on supporting vulnerable adults > 18 years of age in the London Borough of Richmond.

Our pathways consist of:
1. Discharge to assess: minimise hospital admission, support hospital discharge and help patients regain their functional independence in the community.
2. Community Reablement: supporting vulnerable clients in a domiciliary setting using rehabilitation and reablement goal setting.
3. Urgent Community Response (UCR): responding to clients at acute risk of hospital admission, preventing hospital admission and helping to support individuals to remain within the community.

If successful, you will be taking the lead in the development of an integrated team culture, bringing together the different skills, experience and knowledge of team members. Our service is composed of a skill mix of nurses, allied health professionals, dietician and social workers working in the community. You will be central to evaluation, reporting and development of the service and a key contributor to the SWL working groups. You will have the skills required to manage a high functioning, fast paced team implementing and embedding change.

The service manager has a key position within the Division's management structure and is responsible for providing day-to-day operational management and support across a complex integrated service.

They must have the ability to demonstrate strong clinical leadership and deliver service improvements for patients and staff.

The successful post holder will need to have experience of working within a multi-professional team and of working collaboratively with both internal and external stakeholders including demonstrating an understanding of the acute-community interface.

They will ensure the delivery of a high-quality service and demonstrate a high standard of communication and organisational skills. Furthermore, they will need to have a strong commitment to improving services and the ability to influence positive change.
