Full Job Description

We are seeking an enthusiastic, motivated and proactive clinician to join our busy team who is able to demonstrate a high degree of autonomy in assessing patient's mental health care needs, and who can deliver a caring and safe service in a timely and organized way This is an important role for team functioning and ensuring high quality assessments, discharges and signposting for our service users.
Wandsworth Single Point of Access Team (WSPA) provides a borough-based assessment only service for adults aged 18-74, whereby all referrals are channelled through our team, and directed to receive appropriate care either within the Trust or in Primary care. We work closely with the community recovery and support teams, early intervention, crisis resolution & home treatment teams and the AMHP service. We work in close partnership with GP commissioners and other key partners, including voluntary organisations.
This is a friendly and supportive team. You would work alongside the Consultant Psychiatrist Team manager and other clinicians to ensure governance standards are maintained, and support in daily team functioning, thus ensuring that our key performance indicators and quality improvement are met., · To assess and work with mental health service users and signpost them to other services and discharge to primary care where necessary. Any service users needing longer term secondary care and care coordination will be transferred to the relevant teams in the Trust.
· The post holder is expected to utilise community resources, working closely with Primary Care.
· The post holder will ensure user and carer involvement in all aspects of service delivery., · To liaise as required with other disciplines and organisations to ensure that all issues affecting service user care are managed and communicated to a high standard.
· To provide written assessments reports/discharge letters for all allocated clients.
Managerial
· To liaise closely with other services keeping them informed of relevant service issues.
· To provide information and reports as required and accurately document all relevant information regarding clients and staff, in keeping with the Trust guidelines and the NMC standard for records and record keeping.
· To have an awareness of the need to manage resources effectively within determined budgets.
· To assist in the successful induction of new staff and regular supervision of students and junior staff.
· To have responsibility for the health, safety and welfare of self and others. To comply always with the requirements of the Health and Safety regulations.
· To participate in the development of junior staff, acting as a mentor for student nurses and a preceptor to newly qualified staff, ensuring appropriate teaching, support and supervision.
· To participate in clinical/team meetings.
· To enter information onto Rio in a timely manner in line with Trust guidelines and prepare reports as appropriate.
· To implement and contribute to policy and service development as appropriate.
· To carry out responsibilities to promote the smooth running of the clinical area.
· To actively promote and support the ongoing review of Mental Health Practice.
· To participate in the Trust's complaints procedure and critical incident reporting when required and work within the policies and procedures of the Trust.
Professional
· To maintain and improve own professional, clinical, managerial, knowledge and skills by promoting self-growth and development through the in-service training program and other educational program/materials as agreed by the Line Manager.
· To maintain high standards of professional practice and competence in keeping with the professional bodies to whom they are accountable such as the NMC BAOT etc. code of conduct and the scope of professional practice.
· To maintain a record of professional development by use of a personal profile and meet the requirements for continuing practice such as Revalidation or PDP for continued registration.
· Identifying needs/goals through supervision, appraisal and development reviews.
· To participate in nursing research and disseminate the knowledge and results from such research to colleagues.
· To be conversant in and contribute to local initiatives on Clinical Governance, i.e.; Risk Management, Clinical Effectiveness and Local Clinical improvement Groups.
· To be fully conversant with legislation relevant to current practice.
· To initiate/participate in developments and innovations within Mental Health Services.
Training and Development
· To undertake mandatory and statutory training as required by Trust policy, identifying individual and team training needs.
· To contribute and commit to undertaking an annual Development Review/appraisal.
· To undertake personal development as identified in the Personal Development Plan (PDP).

· The post holder will be expected to promote, by example and leadership, a high standard of patient care.
· This is a senior nursing role with immense experience in duty and mental health assessments - it is not suitable for newly qualified nurses or those without adequate experience. The ability to work and make decisions autonomously will be essential.
· The post holder will communicate and collaborate effectively with all agencies and agents involved in supporting adults with severe mental health problems.

Qualifications and registration recognised in the UK
Sound risk management practices
Good experience of working in mental health
Excellent organisational and IT skills
Strong personal skills
