Full Job Description
The RTT and Referrals Coordinator role includes management of the patient journey through the care pathway in accordance with NHS England Referral to Treatment (RTT) guidelines and requirements.
The successful candidate will have good organisational skills and a good knowledge of the RTT guidelines. They will work closely with the Operational Management Team and liaise with the administrative and clinical staff to progress patient pathways and ensure accuracy of waiting lists.
This is an office based position., 1) Process referrals, both paper and electronic, for Plastic Surgery, Maxillofacial, Orthodontics and Oral Surgery, Corneo and General Specialties.
2) To ensure that external referrals received are dealt with in accordance with hospital policy, assessing and responding to each referral in an appropriate and timely manner.
3) Monitor the RTT18 Patient Target List (PTL) on a daily basis in conjunction with all departments, querying and investigating patient pathway status.
4) Investigate and escalate queries concerning more complex patient pathways.
5) Responsibility for validating clock start dates and closing patient pathways when appropriate.
6) Assist all administrative staff with RTT18 queries.

We are fully committed to training and development of the workforce with support for continuing education and learning., + Validate individual patient pathways on the trusts RTT Patient Tracking List (RTT), using the correct application of the suite of NHS RTT rules. This will involve making informed decisions on the information held in the patient clinic letters to ensure that the correct RTT rules are followed and recorded in the trusts IT Patient Centre system.
+ To identify and record information that is pertinent to the patient pathway.
+ Assist the RTT and Appointments Manager with assigned validation of patients and accurately record and report on the reason for any breaches. This will involve updating comments fields in the trusts database.
+ Support the investigation of long wait breaches, e.g. 52 week waiters, recording information of what next steps are required in order for the patient to be treated and the RTT clock to stop appropriately.
+ To review data, data quality reports and patient tracking lists in order to focus resource in the area which will best support RTT performance, and patient pathway delivery.
+ Investigate and respond to any RTT queries from within the trust and externally, highlighting any complex or breaching patients to the Service Coordinators and the RTT and Appointments Manager.
+ To identify any new/additional RTT training available and attend, and participate in these training courses, in line with the needs of the service to maintain, strengthen and develop the skills required.
+ Responsible for identifying and deleting any incorrect pathways, including duplicate pathways.
+ Assist in the development of pathway flowcharts within the department, this will include clock starts and stops.
Referrals Administration:
+ Process referrals, both paper and electronic, for Plastic Surgery, Maxillofacial, Orthodontics and Oral Surgery, Corneo and General Specialties.
+ To ensure that external referrals received are dealt with in accordance with hospital policy, assessing and responding to each referral in an appropriate and timely manner.
+ Using the Patient Management System, register referrals with the correct Referral to Treatment clock start date.
+ To check relevant patient details against all the relevant systems and making updates or amendments on the patients electronic record.
+ Downloading and uploading various documents, including clinic letters, photographs and clinical results from the various referrals systems and the trust's referral NHS.net email account.
+ Check referrals that have been received are scanned into Evolve, with the correct dates being used.
+ Review, identify and register oncology referrals from primary care and from tertiary referrals and send to the Oncology Booking Hub for processing outpatient appointments.
+ Accurately open outpatient episodes, under the relevant services and consultant and/or medical speciality.
+ Assist with all referral queries, both internal and external queries.
+ Using the Patient Management System, register referrals with the correct Referral to Treatment clock start date and clinical priority.
+ To effectively communicate with other trusts to ensure that the clock start date is correct, making use of the Minimum Data Set forms.
