Full Job Description

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients across a Primary Care Network of 7 GP Practices, dealing with a diverse range of health candidate will be based at one site. The role is initially offered on a fixed term contract basis until 31 / 03 / 2025 however this may be extended and will be reviewed ahead of the end date. Main duties of the job The role will include, but is not limited to, surgery and telephone consultations, home visits, administration, complying with the practices contracts (GMS / PMS / enhanced services / DES)., Clinical Responsibilities

  • In accordance with the Practice timetable, as agreed, the post-holderwill be available to undertake a variety of duties including surgeryconsultations, telephone consultations, checking and signing repeatprescriptions and dealing with queries, paperwork, reports and correspondencein a timely fashion
  • Triage patients presenting to the surgery by telephone, online and inperson
  • Making professional, autonomous decisions in relation to presentingproblems, whether self-referred or referred from other health care workerswithin the organisation
  • Assessing the health care needs of patients with undifferentiated andundiagnosed problems
  • Screening patients for disease risk factors and early signs of illness
  • In consultation with patients and in line with current Practice diseasemanagement protocols, developing care plans for health and promoting healthylifestyle strategies and education
  • Manage long-term conditions and patients with complex problems
  • Recording clear and contemporaneous consultation notes to agreedstandards
  • Collecting data for audit purposes and complying with QOF requirements
  • Delivering enhanced services
  • Check, manage & process patient test results
  • Refer patients to secondary care and / or relevant organisations andinternally within the PCN (eg to additional roles members of the PCN team) asrequired
  • Compiling and issuing computer-generated acute and repeat prescriptions(avoiding hand-written prescriptions whenever possible) in line with local andnational guidelines
  • Prescribing in accordance with the Practice prescribing formulary (orgenerically) whenever this is clinically appropriate
  • In general the post-holder will be expected to undertake the duties andresponsibilities associated with a GP working within primary care in a surgeryon a PCN basis
  • Other Responsibilities within the Organisation :
  • Awareness of and compliance with all relevant Practicepolicies / guidelines, prescribing, confidentiality, data protection, healthand safety
  • A commitment to life-long learning and audit to ensure evidence-basedbest practice
  • Contributing to evaluation / audit and clinical standard setting withinthe organisation
  • Contributing to the summarising of patient records and read-codingpatient data
  • Attending training and events organised by the Practice or otheragencies, where appropriate and suitable and relevant for your role
  • Keep up to date with current evidenced based approaches to patient careand service delivery, in line with NICE and national frameworks
  • Contribute to education and training of trainee members of staff andstudents
  • Attending and contributing to practice and PCN meetings

    Experience
  • Essential Experience of primary care through the GP Vocational Training Scheme Qualifications Essential
  • MBBS or equivalent medical degree
  • Full registration with GMC
  • Certification of Completion of Training (JCPTGP / PMETB)

    You will be employed on behalf of the PCN by OurHealth Partnership (OHP).
  • OHP was set up by local GPs who are passionate about providing highquality primary care and using their time and skills effectively to benefitpatients.. We are currently a GPpartnership of 30 practices with 39 surgeries,serving around 280,000 patients in Birmingham,Wolverhampton, and Shropshire. Our head office is based in Cotteridge, but this role will be based at practices within the PCN.
