Full Job Description
For further information on this role, please see the attached detailed Job Description and Person Specification. 2.1 To sustain a high-quality specialist service for the patient, from diagnosis through all stages of the disease and actively provide optimum liaison and continuity of care. 2.2 To use advanced clinical judgement to plan, implement and evaluate care for patients throughout their cancer pathway 2.3 In collaboration with the existing Clinical Nurse Specialists, to formulate and/or revise relevant policies and to regularly review this following change/implementation. To work within the procedures and policies of the Trust. 2.4 In conjunction with the existing Clinical Nurse Specialists, to develop assessment tools that will ensure an appropriate level of nursing intervention so that patients with complex needs are referred to the appropriate specialist/s. 2.5 To manage an agreed caseload, using in depth knowledge of the patient / client group and acting as the key worker where
appropriate. 2.6 To initiate, run and evaluate appropriate group support in conjunction with other relevant professional disciplines.
