Full Job Description
Working as dedicated practitioner to primary care network (PCN) as part of PCN multiagency team alongside mental health treatment practitioners, psychology, psychiatry and social care.
- Provide a combined consultation, advice, triage and liaison function.
- Work with patients to: 1. support shared decision-making about self-management; 2. facilitate onward access to treatment services; 3. provide brief psychological interventions, where appropriate.
- Work closely with other PCN-based roles to help address the potential range of biopsychosocial needs of patients with mental health problems. This will include the PCNs MAT, including, for example, PCN clinical pharmacists for medication reviews, and social prescribing link workers for access to community-based support.
- Operate without the need for formal referral from GPs, including accepting some direct bookings where appropriate, subject to agreement on volumes and the mechanism of access between the PCN and mental health team., Please refer to the full Job Description and Person specification attached to this advert which will provide further information on this role.

- Assessment and management of clinical risk
- Working within a multi-disciplinary team, Experience in working in Primary Care
- Experience of working in a GP Practice
- Working in collaboration with the voluntary sector
- Experience of delivering evidence based psycho social interventions, Able to present sensitive information clearly and concisely, verbally and in a variety of other formats.
- Ability to build and sustain effective working relationships in stressful and demanding situations
- A positive attitude and able to act on feedback
- Ability to communicate effectively in English in both verbal and written formats
- Mental health assessment skills

- Skills for assessing, decision making and problem solving on a range of service planning and clinical issues in the face of considerable adversity and complexity

- Ability to work from home/at alternative work base as required
- Ability to travel/be mobile across the area covered by the Trust, Hold a Registered Mental Health Nurse, Occupational Therapist or Social worker Professional qualification
- Educated to Degree level or equivalent experience
- Diploma level specialist training or equivalent experience
- Evidence of continuing professional development
