Sefton - Adult Social Care North Team £35ph

Case Management: Manage a caseload of adults, providing assessments, care planning, and safeguarding in accordance with statutory responsibilities. DST Assessments: Conduct DST assessments to determine eligibility for Continuing Healthcare (CHC) funding. Collaboration: Work closely with healthcare professionals, service users, and their families to deliver person-centred care plans. Crisis Management: Assist with urgent cases and provide support during a period of high demand in the team.

Qualified Social Worker with HCPC registration. Experience in Adult Social Care, including conducting assessments and care management. Strong understanding of Continuing Healthcare (CHC) and DST assessments. Ability to work independently, manage complex cases, and make critical decisions in a fast-paced environment.

Rate: £35 per hour. Contract: 3 months, with potential for extension based on team needs. This role is urgent due to current staffing challenges, with 6 team members on long-term sick leave and ongoing recruitment for maternity cover and part-time positions.
