I have a mild learning disability, anxiety, and issues with alcohol. I find it difficult to
form relationships and I spend a lot of time on my own. I can often feel anxious,
and I reach for the phone and alcohol to manage this. I do have a good
relationship with my dad who lives locally and supports me when he can.
I am looking for 2:5 hours twice a week, 5 hours in total for a PA support me to
access the community to make ties and connections, to support a voluntary work
opportunity and to encourage physical exercise to include swimming. PA to liaise
with myself and CSS (Community Support Service) on days and times of support
as I rely on co-ordinated support; inconsistencies lead to increased anxiety.
PA needed to prompt and support me to leave my flat and take part in activities
outside of my home. Activities to include cycling (Wheels for All), shopping, visiting
a café, gym, walking, bowling, looking for other activities to take part in.
PA is also needed to support me to engage appropriately with members of the
public. I need support to motivate myself to take part in activities.

Duties include:

Eating and drinking assistance
Other relevant duties as required
