Full Job Description

  • be supporting the Family Practitioner Leads to carry out the Local Authority's SEND duties in relation to co-ordinating Education Health &Care (EHC) assessments

  • producing EHC Plans and carrying out annual reviews

  • work closely with our children and young people with special educational needs and disability and their families as well as a range of qualified professionals in education, health and social care.

  • develop collaborative and respectful relationships, promoting choice and independence and improve outcomes

  • ensure that education, health, and care plans comply with legal, regulatory, ethical and social requirements in addition to the policies, standards and procedures.

    It's a fast-paced role so you'll need to be able to manage your own caseload independently and as part of the team.


  • You will also:
  • Need excellent written and verbal communication skills.

  • Have knowledge of the SEND legislation plus, it's important that you can reflect on practice and improve as a result.
