SEND Social Care Practitioner

This is a unique and exciting role for a motivated practitioner to support the Education, Health and Care process.

An education, health and care (EHC) plan is a statutory legal document for children and young people between the ages of 0 to 25 who need more support than is available or deliverable through special educational needs (SEN) support. The Successful Candidate will play a key role in identifying social care need as part of the EHCP Assessment process.

The role will be based in the Calderdale Multi-agency Screening Team, alongside an Education Co-ordinator. You will build strong links with the Calderdale Special Education Needs Team and will be responsible for gathering Social Care Advice including information from parents during the initial EHCP assessment process.

The successful candidate with have experience and a good working knowledge of Children and Young People with SEND.

You must have good written and verbal communication; you should be able to plan, organise and evaluate your workload; be self-motivated, work well in a team and with other professionals and outside agencies.
