Senior Care Worker

Senior care professional required, to assess service users abilities in their own homes, service users will be new to social care or being discharged from hospital, We are looking for the right person to enable service users to remain as independent as they possibly can, you will be supporting them with personal care, medication administration, light household duties such as laundry and meal prep. You will be assessing them over a period of six weeks to see what support they actually need, you will develop a care plan with their social workers input. and move them forward to the next stage of the service. hours TBC but will include alternate weekends.
