Full Job Description

We are seeking an enthusiastic Band 6 health care professional who will provide specialist care co-ordination within our Specialist Community Forensic Team (SCFT).,

  • To provide clinical support, consultation and advice, working freely within the 4 Tier model.
  • To manage a case load of patients with complex needs including forensic and risk management. With support from other team members provide a robust service and management to our patients.
  • To ensure the delivery of expert assessments and interventions and ensure care is delivered effectively and to a high standard. Ensuring that everybody involved the case are involved including other professionals, outside agencies, carers and family.
  • Ensure the delivery of consistent care, professional boundaries are maintained and based around evidence based in accordance with best practice.
  • To ensure we are able to clearly identify risks and any safeguarding concerns and escalate accordingly.

    You will have experience of working with people with severe and enduring mental health problems including forensic experience and have a qualification equivalent to degree level in a clinically relevant discipline.
