Senior Case Worker (Physical Disabilities)

Joining our Physical Disabilities Duty Team as a Senior Case Worker, surrounded by a supportive team of skilled practitioners, you'll be person-centred and committed to working in a strengths-based way.

Our Duty Team triages contacts from GPs, care agencies, out of hours, the Contact, Assessment and Resolution Team (CART), the Adults' Multi Agency Safeguarding Hub (MASH), families, and individuals themselves. We prioritise urgent cases and gather further information to make decisions on whether cases require allocation within the Physical Disabilities service or passing onto another Adults' Health and Care service. We also provide signposting to other agencies where appropriate, as well as completing funding applications and recording assessments and support plans. Most of our work is done via telephone and email, however we sometimes need to visit individuals to carry out welfare checks.

What You Will Be Doing:

• Triaging incoming contacts to the Duty Team, and understanding when to escalate more urgent or complex cases.


• Completing tasks that have been allocated to you by the Team Manager or Social Worker.


• Providing mentoring and advice to Case Workers and supporting the team with more complex cases where needed.


• Preparing clear and concise reports.
  
  Good understanding of the Care Act 2014, Mental Capacity Act 2005, and other relevant legal frameworks.


• Strong communication and organisational skills.


• Strong team working skills for sharing learning and providing support and advice to colleagues.
