+ Post holder to work flexibly across the Children's Community Nursing service, delivering high quality, evidence based nursing care for continuing care children (ICB commissioned packages of care) and young people with short and long term conditions including supporting children with life limiting and life threatening conditions. + To liaise with the staff in the school regarding any changes in the care plans of the ICB funded children and young person. + To keep up to date with members of the MDT team and with families, parents and carers. + To accept responsibility for own professional practice as a reflective, accountable clinical practitioner. + To provide nursing interventions to meet the complex medical health needs of young person in school including supporting the young person on transport to and from school. + To work within the Children's Community Nursing Service, delivering high quality, evidence based nursing care for children and young people with short and long term conditions, including life-limiting and life threatening conditions that receive continuing care funding. + To meet these health needs of the identified children by working in liaison with multi-agency partners to promote collaboration and effective communication. + Please note that driving, and being a car owner is an essential requirement for this role as clinical interventions are undertaken in the community., + Post holder to work flexibly across the Children's Community Nursing service, delivering high quality, evidence based nursing care for children and young people with short and long term conditions including supporting children with life limiting and life threatening conditions, preventing hospital admission and facilitating early discharge. + To facilitate specific training and support strategies for those personnel involved with children and families, with a variety of healthcare needs, whilst also acting as a specialist resource for all colleagues, within educational and community settings. + Participate in the developments of the Children's Nursing Team in line with best practice outlined within research, national guidelines and local governance arrangements. + To actively participate within specific medical consultant led outpatient clinics within the geographical area. + To support the Team Leader in the effective daily management of the patient / team caseload + To accept responsibility for own professional practice as a reflective, accountable clinical practitioner. + Ensure effective line management of staff CLINICAL & PROFESSIONAL RESPONSIBILITIES + Receive referrals into the service and actively participate in the assessment of children requiring nursing management/, within any educational and community setting. + Plan and deliver individual programmes of specific care for children within a range of care and educational settings, who have a variety of healthcare needs in a competent manner that recognises own limitations and seeks appropriate supervision. + Possess advanced clinical skills to closely monitor the child's progress and improvement, and to detect deterioration promptly following the agreed care pathway to medical reassessment. + Respect the rights of children and young people and ensure verbal and when appropriate written consent is obtained prior to carrying out any clinical procedures. + Participate in the discharge planning process, both to and from the service, whilst maintaining effective communication links with colleagues from all relevant professions. + Deliver high quality care and training, by ensuring both clinical practice and all verbal and written information is evidence based. + Deliver specific health education, health promotion and illness prevention advice to fellow service providers and children / families. + Actively participate within Audit, Paediatric Benchmarking and Essence of Care processes. + To respect family's cultural, spiritual and religious practices and work in partnership with families in promoting parent participation and child / family centred care + Participate in the proactive planning process for the transitional period, prior to young people transferring into adult care services. + The post holder shall fully participate and where necessary lead on the Common Assessment Framework, to ensure that all children with additional needs are provided with a co-ordinated multi-agency response to meeting their needs. + Foster and maintain effective communication channels, by undertaking the role of Named Nurse, with the children, families/carers and all multi-disciplinary / multi-agency professionals involved with the child and family. + To initiate, develop and maintain effective liaison with other community nurses / teams, secondary and tertiary specialist services. + Provide accurate and regular reviews of the child's physical, emotional and spiritual needs and on the progress of their condition / diagnosis. + Ensure that verbal communications and written documentation are maintained and are clear, relevant and concise, in accordance with Nursing and Midwifery Council's (NMC) Standard's of Records and Record Keeping and Trust policy. + Actively promote the service's philosophy and values to support the child and family, whilst acting as an advocate for the child and family at all times. + To have knowledge into the organisational culture, policies and politics, in order to have an understanding of how the organisation works and how practice / service delivery can be affected. + Where applicable, to prescribe under the V300 non-medical prescribing safely and effectively. SAFEGUARDING RESPONSIBILITIES To be competent at safeguarding children and implementing the Child in Need and Child Protection procedures as appropriate includes the following skills, competencies and knowledge: - + Be able to recognise child abuse. + Be able to document accurately child protection concerns according to Laming recommendations, differentiating fact from opinion + Know who to contact / inform / seek advice from; know how to contact them 24 hours a day, seven days a week, know when to contact them according to Trust and Local Safeguarding Children's Boards policies and procedures + Understand the next steps in the child protection process + Know when and how to act, when and how to refer and the importance of following up a referral. + Understand which groups of children are at risk of harm or neglect. + Know how to share information - in writing, by telephone, electronically or in person + Know who to share information with and when, understanding the difference between information sharing on individual, organisational and professional levels. + Know what to record, how long to keep it, how to dispose of records correctly, and when to feedback or follow up. + Have understanding of own (and others') professional roles and boundaries, including the Named professionals, trust managers and social workers. + Knowledge of the implications of key national document/reports. + Understand the assessment of risk and harm. + Understand multi-agency frameworks, investigations, working and child protection assessment processes including the use of the Common Assessment Framework / investigation/working. + Be able to present child protection concerns in a child protection conference. + Demonstrate ability to work with families where there are child protection concerns. + Understand forensic procedures/practice. + Be able to advise other agencies regarding the health management of child protection concerns. + Ability to contribute to serious case reviews or individual management reviews. + Understand child protection investigation and the basics of forensic procedures. + Aware of Local Safeguarding Children's Board and it's remit. + Know how to access the child index register., + Facilitate the training and ongoing support of children / families / carers and all professional colleagues, according to their level of knowledge, ability and willingness to participate in care delivery, including the assessment of their knowledge and competence. + Have a particular interest in ventilation of children, and the training for staff currently within the team and trust as well as non-medical carers. + Assist in the support, training and assessment of student nurses, ensuring their learning outcomes link theory to clinical practice. + Participate in developing networking links with colleagues within all relevant health, education and social care colleagues. + Within the team, share new skills, knowledge and expertise, which is proven best practice and evidence based. + To hold or to be willing to work towards the Specialist Practitioner Qualification in Children's Community Nursing. + Participate in the development and evaluation of specific training information and material's, in order to disseminate and promote evidence-based care to all relevant colleagues and families. LEADERSHIP RESPONSIBILITIES + Receive and assess any referrals and be responsible for the planning and delivery of high quality, evidence-based care for children with a variety of healthcare needs. Liaise with service and clinical coordinator as appropriate. + Participate within staff performance reviews and the formation of personal and team development plans. + Participate in the development and review of the services Care group and organisations policies procedures and guidelines + To be responsible for the most effective and efficient use of resources and equipment and to rapidly identify any replacement needs and resource issues. + Participate in the continuing development of induction, orientation and preceptorship programmes for all permanent team members, bank staff and student nurses. + To be responsible for managing own daily workload and planning, prioritising and delivering care effectively, whilst identifying and managing any risks to the quality of care provided. We are seeking a dynamic and motivated Senior Nurse to join our Children's Community Nursing Team in the Community of St Helens and Halton areas. You will be based in Lowe House HCRC in St Helens and will be expected to travel across the boroughs. There will be lots of opportunities for learning new skills; the successful candidate must be comfortable in working in patients homes, liaising with other hospitals and preparing care plans for children in the community. There will also be opportunities to work directly with complex needs children within the school setting. An Enhanced DBS with barred lists for Children is required for this post.
