Full Job Description

? Act as the point of clinical contact for all referrals to Bromley Healthcare. This will include the triage and prioritisation of referrals as well as the provision of specialist advice to both referrers and internal clinicians within Bromley Healthcare (BHC).

? Be responsible for managing referrals into the appropriate clinical team,demonstrating sound clinical decision making and the use of EMIS and the Electronic Patient Record (EPR) systems and processes to provide clear evidence of effective management. Working with the CCC and service leads to effectively co-ordinate staff to provide the most effective patient care.

? Manage the safe transfer of care between care providers and makeappropriate referrals to other care providers outside of Bromley Healthcare

? Make consistent judgements requiring complex fact analysis, interpretation and comparison of options and ensure clinical reasoning is reflected accurately in case records.

? Balance clinical risk and clinical decision making against context, ensuring patients receive safe, effective and timely care.

? Whilst utilising your allied health care professional expertise and clinical skills, you will enhance the experience of all appropriate patients under our care to access and use the facilities/services available in the community.

? Enable patients to be cared for safely by remotely supporting established care packages at home or in their place of residence.
