Full Job Description

Ageing & Health are working together with the Integrated Care System in Lambeth & Southwark (Lambeth Together & Partnership Southwark) to transform frailty services. The advertised post offers an exciting opportunity to contribute to this integrated transformation work developing skills in frailty care as well as skills in service innovation and integrated working. The post holder will work in delivering Comprehensive Geriatric Assessment (CGA) as well as end of life care support for older people in the community setting. This post is suitable for ST4-ST7 level in Geriatrics/General/Acute Medicine/Palliative Care or senior GP trainees. Candidates will have an interest in advancing their skills in managing complex older people in the community and in MDT working. Although the post is out of programme (OOP), the activities have been designed to meet the educational learning objectives across the relevant curriculums. The post holder will develop skills in quality improve and service
innovation. There is opportunity to participate in teaching and/or research depending on the fellow's personal interests and development goals. Trainees are encouraged to discuss their application with the Training Programme Director prior to application to ensure their support.
