Full Job Description

We are seeking an enthusiastic, patient-focused, independently motivated pharmacist to join our Community Response & 2 hour Urgent Response Teams. The post holder with be co-based with the teams, currently at Cobalt Exchange in North Tyneside.

We provide a proactive and collaborative, high quality medicines optimisation service for patients in their own homes working closely with colleagues in health and social care across both the primary and secondary care sectors.

The role will provide a mix of face to face, patient-centred consultations and telephone support to patients, family members and carers as well as involvement with multidisciplinary team meetings. In addition, provision of responsive medicines management advice and support within the 2HUCR service providing support to patients with urgent medication needs., Working within professional boundaries to provide person-centred, evidence-based structured medication reviews for residents in their own homes.

Targeted reviews of frail elderly patients in their own homes to help minimise risk at transfer of care including those patients who have recurrent falls,complex polypharmacy, swallowing difficulties and those taking high risk medicines.

Close working with the multi-disciplinary team, which is likely to include attendance at MDT meetings. Interacting with a wide variety of health/social care professionals including; GPs; community nurses/matrons; community therapy teams; specialist secondary care consultants (e.g., geriatricians, Parkinson's disease specialists); the old age psychiatry team; and pharmacy colleagues in community pharmacy, GP practices and the hospital setting.

Patients are reviewed holistically to identify and classify their medication risk and pharmaceutical care plans are implemented to reduce their risk of poor outcomes from polypharmacy and inappropriate prescribing

Provision of expert advice, information and recommendations on medicines and prescribing, including support to NMPs within the service, providing clinical governance in areas such as antimicrobial and opioid stewardship.

The role is patient-facing, with reviews conducted through a combination of telephone, face-to-face and video consultations with patients/residents and their families/carers.

We need an experienced pharmacist with a clinical diploma or equivalent clinical pharmacy experience, and a non-medical prescribing qualification would be preferable.

We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital.

Our Community Services covers an area of more than 2,500 square miles across North Tyneside and Northumberland. Our joined-up care means patients can access specialist services without going into hospital. We provide urgent care when you need it most, keeping you safe and well at home to avoid hospital admission.
