Full Job Description

This is a innovative new role that will focus on implementing and leading pharmacist-led clinics in the management of hyperlipidaemia using novel medication. The post holder will join the forward thinking and progressive Pharmacy department at Airedale General Hospital, on the outskirts of Skipton., 1. To implement and lead pharmacist-led clinics for the treatment of hyperlipidaemia.
2. To provide a specialist clinical pharmacy service to clinics and specialist areas, with a particular focus on endocrine and diabetes
3. To provide a daily clinical pharmacy service to designated wards to support the clinical pharmacy service.
4. To provide clinical and financial management on Continuous Glucose Monitoring (CGM) and Hybrid Closed Loop treatments.
5. To support the clinical management of patients who receive endocrine and diabetic treatments from the Trust, who out outside of the West Yorkshire Integrated Care System.
6. To lead a range of schemes for the Medicines Optimisation Waste Reduction Programme
