Full Job Description

  • This post will provide GP clinical pharmacy services to Cauldwell Medical Centre, and can be onsite or remote


  • The Primary Care directorate at ELFT have an exciting opportunity for an enthusiastic, motivated, and innovative Senior Clinical Pharmacist to join our Team.

    We are looking for an individual who will support medicines management and be a clinical leader for medicines optimisation within Cauldwell Medical Centre.
  • Work as part of a multi-disciplinary team taking responsibility for

  • areas of chronic disease management within the practice

  • Undertake clinical medication reviews to proactively manage

  • patients with complex poly-pharmacy, especially for older

  • people, people in residential care homes and those with multiple co-morbidities

  • Provide primary support to general practice and GPSU staff with regards to prescription and medication queries

  • Clinical leadership of medicines optimisation, plan and organise quality improvement initiatives, appropriate specialist services and manage some aspects of the quality and outcomes framework and enhanced services

  • The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

  • Professional registration with GPhC

  • Master's degree in pharmacy (or equivalent)

  • Experience of working as a qualified, registered pharmacist in primary care, community or hospital pharmacy

  • Independent prescriber

  • Completed the Primary Care Pharmacy Education Pathway of the CPPE

  • Awareness of general practice and the agenda for medicines optimisation

  • Good clinical pharmacy knowledge including terminology
