Full Job Description

An exciting opportunity has arisen for two experienced, forward thinking and enthusiastic clinical Pharmacists to join a new innovative specialist Hospital@Home Frailty Team.

We are in the process of developing a new multi-disciplinary team and pathways which involves the identification and then management of frailty patients in their own homes. The service will include a frailty same day emergency care unit within the hospital.

These posts provide the opportunity to shape and develop the pharmacy service from the beginning.

The role will be based either at Darlington Memorial Hospital or University Hospital North Durham and the service will run Monday to Friday during core working hours. There is no weekend working or on call commitment associated with the role.

You will belong to a team that is committed to supporting staff health and well-being as well as professional development in order to help you achieve your full potential., The post holders will, as part of a multi-disciplinary team, provide expert medicines support to the Hospital@Home team.

You will be involved in setting priorities, developing guidelines and pathways, education and training around medicines, audit, governance and medicines safety across primary and secondary care.

You will lead in the review of medication in frailty patients, developing pathways which promote appropriate prescribing and deprescribing.

There will an opportunity to develop your clinical skills and build on your knowledge to independently review these patients as in-patients, in same day emergency care or within their own homes. You will become familiar with frailty syndromes and components of a geriatric assessment.

You will also be responsible for other members of pharmacy staff in the Hospital@Home team including pharmacists and pharmacy technicians. We also have well established pharmacy teams in acute frailty, acute medicine and same day emergency care who you will work closely with.
