Full Job Description

Using GP systems (EMIS and System 1) you will support practice sites with medicines related tasks, this may include but is not limited to :

  • Discharge support (meds rec)

  • General med reviews

  • Supporting patient requests for acute and repeat meds

  • Structured Medication reviews

  • Safety audits

    You will support practice staff with prescription and medication queries. Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.

    You will support with prescribing practices across the PCN, providing clinical leadership on medicines optimization and quality improvement.

    The right candidate for the role will have completed their IP qualification and have completed their CPPE pathway. The right candidate for the role will have at least 1 years experiecne in primary care in a leadership role
