Full Job Description
You will be the medicines optimisation lead for the local Proactive
Care multidisciplinary community teams in your specified area(s).
As a core member of these teams, you will visit adults with multiple long term conditions in their own homes for patient-centred structured medication reviews (SMR), reviewing prescribing and making recommendations to improve patient outcomes focusing on quality and safety. You will be liaising with primary care pharmacists, community pharmacies and others involved in patient care as required to implement medicines optimisation.
You will be supported by a small team of experienced pharmacists/technician as well as the wider trust pharmacy team (the medicines management team (MMT)).
Your development and career progression is also important to us - to start with you will need to have a Clinical Diploma or evidence of robust relevant CPD, a knowledge of prescribing in the frail elderly population is desirable but not essential as we can support your development. If you are not already a prescriber, we will support you to undertake a prescribing course once established in role.
Looking to the future the proactive care pharmacy service is part of an expanding, dynamic and proactive Medicines Management team with roles across East and West Sussex including in-patient, care home, advanced and specialist pharmacy roles, so there will be opportunities to further develop your career in the future.

If so, we are looking for highly enthusiastic, proactive and self-motivated Senior Clinical Pharmacists to join our Medicines Management Team., To be considered for this exciting opportunity you will need excellent communication and interpersonal skills and proven team working abilities. You should be able to work autonomously and manage your time effectively by prioritising your own workload, and clinical issues to make
effective patient centred interventions to improve care and outcomes for patients. You will need to be resourceful and make complex clinical decisions to provide advice, including where there is ambiguity. You will recognise the importance of key performance indicators in demonstrating
the differences made to patient care and patient outcomes.

Working hours are between Monday to Friday, no weekends or on call. Flexible working may be considered.
