Full Job Description

The Community Nursing Service aims to provide an excellent standard of person centred planned and unplanned nursing intervention to patients in their homes and is part of an integrated service that supports patients to remain in their home environment. As a County Wide Community Nurse you will work closely with the multi-disciplinary team including GPs, Practice Nurses and Specialist Nurses in order to support the service delivery having a high focus on quality outcomes and improving the patient experience., As both an organisation and a service we are actively involved in both local and national innovative projects around improving quality of care and integrated working. We promote mobile working technology to aid staff to provide a safe robust service for the patients. We have an extensive program of training and skills development available and actively nominate and encourage our staff to apply for national awards, such as Queen's Nursing Institute, RCN and Nursing Times awards. Motivated staff are encouraged to complete extended skills training such as the DNSPQ, Advanced Clinical Assessment Skills and Non-medical Prescribing (V300).

We are currently seeking a dedicated, caring, compassionate & highly motivated Community Nursing sister (Band 6) to join our Comunity Nursing Team. Applications are welcomed from nurses who have 2 or more years' experience post qualification and previous roles working as a community nurse will be advantageous.
