Full Job Description

The post holder will work in the Basildon & Brentwood Older Adult Community Mental Health, Dementia & Frailty service. The post holder will work with other team members and other system colleagues to support those with functional mental health issues, including those with serious mental illness (SMI). Providing person centred care and working within the recovery model approach enabling patients to work towards their goals and supporting the maximisation of health outcomes. This will include both psychological and physiological observations and the promotion of healthy lifestyles for our patients, signposting to relevant physical health interventions as required. The post holder will assist in enabling patients to access the support they need to address their physical and mental health needs and promote recovery., If you are joining our Trust in a fixed term role, please indicate on your New Starter Paperwork that you wish to join our staff bank.

  • The post holder will work as part of the multidisciplinary team
  • The post holder will contribute to assessment of both physical and mental health needs. Focusing on the wellbeing of those with serious mental illness, and demonstrating an understanding of the relationship between physical and mental wellbeing
  • To work with other agencies to deliver a systems approach in ensuring patients mental and physical health wellbeing needs are met
  • The post holder will undertake annual physical health checks of patients who have been identified as having severe mental illness. They will take and record their blood pressure, pulse, weight, BMI, venepuncture for required blood tests
  • The post holder will arrange physical health assessments with the service user both at home, in clinics, and at GP surgeries; taking a proactive approach to engaging patients with SMI in the importance of annual physical health reviews
  • To provide information on health promotion including healthy eating, sport and exercise and smoking cessation
  • To work with patients and relevant staff to develop care plans and complete the electronic physical health assessment form on electronic health record; ensuring information is shared with patients and GPs.
  • To promote positive mental and physical health to patients, carers, families and staff to support patients to consider all needs and identify their own physical health goals and to support monitoring goals identified with other members of the team

  • The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving our population's health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. The academy models a 'one workforce' cross system approach to the onboarding, induction, education and experience of Healthcare Assistants. To support this 'one workforce model' the successful candidate for this post will be required to attend our 5 day (Monday to Friday) face to face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of two of the HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process. Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and will provide opportunities to enhance skills, knowledge and experience. Further information will be provided post interview. This will be followed by an additional 5 day (Monday to Friday) EPUT corporate / mandatory training welcome. (EPUT Welcome will be varied between face to face and virtual delivery).
