The Hounslow Borough Team in North West London ICB is seeking a highly motivated and enthusiastic individual to join a dynamic team to provide innovative ideas and solutions by collaborative working with partners to help children and young people in the community.

The Senior Delivery Manager for Complex Care Band 8a (CYP, LDA &AMH) sits within a system wide approach to supporting people in the community whenever possible, making sure we have a preventative approach and work as a joint system. It is a partnership between ICB, Council, WLT, HRCH and Chelsea & Westminster. It is a whole system approach. Most of our system is designed on evidence and data and focuses on supporting people in the community and reducing the number and length of stay of in acute beds.

You will lead on a number of interagency meetings; e.g. DSR, LAEPS, CETRS, working jointly with partners to find the best possible outcomes for Children and Young People., The post holder will be required to provide effective and high quality leadership, programme and project management skills. The successful candidate will be reporting to and supporting the Assistant Director of Children Commissioning.

NHS North West London Integrated Care Board (NHS NWL ICB) is a statutory body responsible for planning and allocating health and care resources to improve the lives of people in eight boroughs across North West London: Brent, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow, Royal Borough of Kensington and Chelsea and Westminster.

All ICB roles work collectively to ensure our residents and patients have access to the right services when they need them as well as focusing on prevention and health promotion, supporting residents to take greater care of their own health and live longer, healthier lives.
