Full Job Description

  • To have a clear understanding of the vision of the Trust and be able to translate this into a local context for operational implementation within the Diabetes SpecialistNursing Service within your defined locality area.
  • To assist the Service Manager in promoting the vision and contributing to the establishment of an ethos of modern, customer orientated services and translate that into a culture of effective patient/user delivery in Locality setting.
  • To work with the Diabetes Specialist Nurses & Service Manager and other team leaders/managers within the county to proactively support the development and redesign of care pathways embracing the concept of care closer to home and expanding/enhancing multi-disciplinary/multi agency working for people with diabetes.
  • To assess, plan and implement and evaluate evidence-based nursing care for people with complexneeds for their Diabetes management.

You will be able to demonstrate a real enthusiasm for community Diabetes Specialist Nursing, as well as a desire for on-going professional development. Ability to travel independently to fulfil the requirements of the post is essential.'
