From 4 April 2024 overseas workers requiring a general Skilled Worker Visa (such as data analysts, IT and finance professionals, etc) but sponsored by an NHS organisation will need to meet the new salary threshold of £38,700. In addition, applications to the Health and Care Visa route for those occupations paid by NHS bands that are on the immigration salary, a list of skilled jobs which have lower salary requirements, will need to meet the national pay scale for their role or the new minimum salary threshold of £23,200 (whichever is higher). + To assist the Service Manager in promoting the vision and contributing to the establishment of an ethos of modern, customer orientated services and translate that into a culture of effective patient/user delivery in Locality setting.
+ To work with the Diabetes Specialist Nurses & Service Manager and other team leaders/managers within the county to proactively support the development and redesign of care pathways embracing the concept of care closer to home and expanding/enhancing multi-disciplinary/multi agency working for people with diabetes.
+ To assess, plan and implement and evaluate evidence-based nursing care for people with complex needs for their Diabetes management.

We are looking for an experienced Senior Diabetes Specialist Nurse, to join a dedicated, supportive and inclusive team to work in our busy, professional and dynamic service. You should possess commitment to autonomous practice , interdisciplinary team working, structured education and innovative delivery of excellent standards of care. You will be able to demonstrate a real enthusiasm for community Diabetes Specialist Nursing, as well as a desire for on-going professional development.
Ability to travel independently to fulfil the requirements of the post is essential.'
