Full Job Description

As a Senior Practice Nurse you will have extensive experience as an autonomous Practitioner and non- medical prescriber in a General Practice environment. You will have an excellent understanding of the Quality Outcomes Framework and other National and local quality targets. You will be able to deliver enhanced clinical care underpinning this knowledge to manage complex chronic disease presentations whilst recognising the complexity of operating in multi-professional and multi-agency environment and the need for interdependent decision-making. At this level the clinician will be highly experienced in their field with an expectation that you will have also studied at an advanced level. You will be expected to provide training, support and supervision to staff and to participate clinical audits and research projects and implement changes as required, including the development, and updating of practice protocols / guidelines and procedures locally., Able to undertake complex general practice nursing interventions for a wide range of general practice issues and teach and develop other staff to enhance their practice to manage future situations. This will entail being able to demonstrate not only practical knowledge but also a critical understanding of the range of theories and principles that underpin the general practice nursing approach.

We require applicants whose native language is not English to demonstrate their competence in the English language.This can be demonstrated by:Providing Certification of English proficiency. During interview should you be invited to attend., Will be able to assess capacity for informed consent and support other staff to develop this skill.

Will have highly developed specialist knowledge and understanding of LTCs, health behaviour and interventions to improve health outcomes, including the use of technologies to support patients at home.

Able to manage clinical care at the highest level independently, from beginning to end, for example from when an individual presents through to the end of the episode, which may include admission, referral or discharge or care at home.Advanced knowledge and skills in therapeutics to prescribe effective pharmacological and non- pharmacological approaches for the management of LTCs and assess patient concordance.

Able to respectfully challenge practice, systems and policies in an objective and constructive manner.

Participate in opportunities to influence local policies, pathways and procedures.
