Full Job Description

An exciting and unique opportunity has arisen for a Band 3 Health Care Assistant to join our Community Cardio-Respiratory Team to play a key part in our digital cardio-respiratory virtual wards.

The virtual wards allow patients to be discharged safely from hospital and then be closely observed using remote monitoring technology in the community.

This role is part of the community team in LPT but will involve working with the team on the cardio-respiratory wards at Glenfield Hospital to help identify suitable patients for the virtual wards prior to their discharge from hospital. The role also includes working in the community to complete follow up home visits for COPD patients.

The main aspect of the role will be to work with patients prior to discharge to set them up with and teach them how to use the virtual ward technology to enable them to use the digital system and safely enter their information and details once they are at home.

Applicants need to be motivated, enthusiastic with excellent communication skills, be able to demonstrate they are hardworking and conscientious as well as friendly and approachable.

An excellent understanding of using IT is essential as this is a vital aspect of this role.

The successful applicant will be working within the Community MDT and be supported by senior staff in the respiratory and heart failure services.

The post is permanent and will involve travel across Leicestershire.

The team provides the service over 7-days a week., The principle role of the Senior Healthcare Assistant is to assist the healthcare team in the provision and delivery of programmes of patient care under the indirect supervision of registered practitioners. This includes:

  • Taking physical or psychological measurements from named clients/service users as delegated by the team.

  • Supporting clients/service users in maintaining their health and wellbeing.

  • Undertaking specified and delegated clinical and therapeutic activities.

  • Under the supervision of healthcare professionals assist the healthcare team with the planning, organising and provision of clinical care.

  • Ensuring high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals.

  • Assisting Registered Professionals in the implementation and evaluation of individualised care plans.

  • Accurately recording patient information.


  • It is essential that you hold a full UK driving licence and have access to a vehicle., working patterns in line with our flexible working policy.
