Full Job Description
We are seeking to recruit an experienced, competent, and motivated Senior Healthcare Assistant to work alongside the two Hepatitis C specialist nurses. This is a rare and exciting opportunity for any healthcare professional with an interest in this field.
The successful candidate will contribute to the implementation and delivery ofhigh-quality patient care within a professional team. Working with the 2 Clinical Nurse specialists to encourage patients/service users to engage with testing and treatment of Hepatitis C. To work with the South Yorkshire Operational Delivery Network (ODN) to deliver NHS England's elimination plan for Hepatitis C.
You will be working in a variety of settings, which may include the 3 Doncaster prisons, homeless hostels, drug services and probation, to engage with a diverse and complex client group utilising an inclusive and non-judgemental approach.
A large proportion of this role will be screening for Hepatitis C and case finding, working alongside other agencies and community groups in order to identify any new positive patients, educate and refer onto the Specialist nurses for treatment., Ø Undertake agreed activities as delegated by a registered nurse, with patients / service users under the direct /In-direct supervision of the qualified staff.
Ø Contributes to the implementation and delivery of individualised evidence-based care.
Ø To maintain accurate records electronically, record new problems as they are identified and report all actions to nurse in charge.
Ø The post holder is required to take personal responsibility to incorporate the 6 C vision of care into their care delivery
Ø Provide feedback of current patient / service user status to appropriate members of the-professional team.
Ø Develop a rapport with patients / service users and encourage them to participate in their plan of care
Ø Work in partnership with patients/carers and multi-agency teams to promote Health and independence
Ø Assist the relevant professional during assessment / treatment / intervention where required
Ø Support the patient / service user to make use of available services and facilities.
Ø Organise and run testing events and in partnership with other agencies
Ø To obtain follow up bloods from patients with poor venous access
Ø Following Health and Safety regulations, promote where possible a safe Environment
