Full Job Description

We are offering an exciting opportunity to join Acute Care at Home, Virtual Ward as a Senior Advanced Liaison Practitioner on a 6 month Secondment.
Our Virtual Ward is a new and innovative service which offers face to face acute care in the patient's own home. The Virtual Ward has an agreed admission criteria which works in partnership with supporting care services and the aim of the service is to reduce length of acute hospital stay and/or avoid hospital admission; this leads to quicker recovery and reduces disruption to patients', and their carers' lives. This means we can proactively manage people living with frailty at home, and for those that have a sub-acute episode we can improve a patient's experience and outcomes. In addition, this means we have more space in our hospitals for acutely unwell patients and reduces the pressure on urgent care.
Working with University Hospitals of Derby and Burton (UHDB) and University Hospitals of North Midlands (UHNM), the role is to act as the Senior Advanced Liaison Practitioner between the Acute Trust and Virtual Wards, in a "pull" function, to ensure the correct utilisation of Virtual Ward beds.
This role is a face to face service working 5 days per week Monday to Friday., The role is designed to support the safe transfer of care from Acute Hospital wards, portals and departments into a Virtual Ward. Working closely with the Virtual Ward Matrons to deliver Case Management, Bed Management and Acuity Management of the patients suitable for Supported Early Discharge.
Identify suitable patients who may need ongoing Virtual care within their own homes, reviewing clinical diagnostics and decision making to ensure the safe transfer of care. Ensuring Medical Management plans are appropriate and in place for discharge.
Educate and empower hospital staff to identify suitable patients who meet the criteria for Virtual Ward care.
Supporting the Virtual Ward Matrons with the implementation of a Virtual Ward case management system.
Develop a comprehensive and cohesive plan for supporting discharge flow which meets the strategic direction of the service/team and minimises unnecessary disruption to stakeholders involved in the process and is operationally sound.
